We, Mani Kant Choudhary and Sarika Kumari, are permanent residents of Gandhipuram East Rupaspur PO Dhanaut Via Sahaynagar PS Rupaspur, Patna-801506, Bihar, India, and presently residing at 1716 Ontario Ave APT 204, Naperville, IL 60563 USA, hereby declare that the surname Choudhary has been added to the name of our minor child, Shreyasi. Henceforth, our child’s name shall be known as Shreyasi Choudhary for all official purposes.
